Medicaid Redesign Team Health Homes Supportive Housing Program


Description:

The New York State Department of Health (Department), Division of Program Development and Management, Bureau of Social Care and Community Supports announces the availability of $86,876,000 statewide for five years in New York State (NYS) funding for up to 17 awards that will provide permanent rental subsidies and housing tenancy services to provide housing for vulnerable homeless Medicaid members. The rental subsidies and services provided under the Health Homes Supportive Housing Program are intended to be a means to provide affordable and stable housing and services, thereby improving access to health services and connection to health system.

At least one (1) award will be made to each Service area listed in Table 1 (of the RFA) to ensure that the program continues to serve current participants. Some counties may have more than one award (see Table 1). Applicants may apply to serve more than one (1) Service area but must submit a separate application for each Service area. Grantees must have a transition plan in place to accept and serve any existing Health Home members housed under this program.
