The Long Island State Veterans Home (LISVH) will receive Request for Qualification (RFQs) for Architectural Design Services for project number 040425EPOFFICES titled Long Island State Veterans Home (LISVH) East Pavilion Offices & ADHC Bathroom Conversion Design until 3:00 p.m. local time on Friday, April 25, 2025 at Long Island State Veterans Home, 100 Patriots Road, Stony Brook, NY 11790 in the Purchasing Department of the East Pavilion. RFQs may be hand delivered or mailed to the above location and must be received at such time. Any RFQ received, whether hand delivered or mailed, after the opening time will not be accepted and will be returned unopened to the Consultant.
Scope of work to include:
The Long Island State Veterans Home (LISVH) is located on 25 wooded acres on the campus of Stony Brook University across from the University Medical Center. Opened in 1991, the LISVH serves a predominantly male geriatric population. The LISVH is a 350-bed skilled nursing facility as well as 50 slot Adult Day Health Care (ADHC) program providing service for honorably discharged Veterans, their spouses and Goldstar parents.
The LISVH is seeking a firm to redesign two (2) existing offices, create one (1) new office and convert one (1) bathroom to an office. The current areas and offices occupy roughly 700 total square feet in the basement level in the LISVH.
The approved firm will evaluate the current areas and provide several design options and will be evaluated by the LISVH Administration, LISVH Engineering Department and Construction Project Manager.
Project Budget is approximately $425,000
Six (6) bound copies of the RFQs and one (1) electronic copy on an USB flash drive must be submitted in accordance with the instructions contained in the Information for RFQs.
It is the policy of the State of New York and the State University of New York to ensure usage of Minority (MBE) and Women-Owned Business (WBE) enterprise participation in this project by contractors, subcontractors and suppliers, and all bidders are expected to cooperate in implementing this policy in order for the LISVH to be in compliance with the Governor’s directive for increased participation of 20% (10% MBE, 10% WBE). A portion of the project funding is Veterans Administration related therefore requiring the completion and submission of Federal Debarment forms with each proposal. Consultant must have a minimum of five (5) years of experience completing projects within a hospital/skilled nursing home of similar size with associated rules and regulations. Work portfolio to be included in RFQ package.

The State University of New York reserves the right to reject any or all bids.
