Oral Surgery ServicesDescription: Contractor(s) Responsibilities - Services Included 1) The CONTRACTOR must be available two (2) to four (4) days per month (census dependent), Monday through Friday, (excluding Holidays) between the hours of 8:00 AM and 3:30PM. 2) The CONTRACTOR will provide dental/oral surgery services, at their place of business, for an estimated maximum of 6 patients per month. This amount could be depending on needs of the facility. The schedule will be coordinated through the Capital District Psychiatric Center (CDPC), which allows for direct consultation with the facility staff. 3) The CONTRACTOR is responsible for radiological imaging and diagnoses, interdisciplinary consultations, extractions, biopsies, treatment of fractures, medical assessments and tests as required for the practice of oral and maxillofacial surgery, implants, local, parenteral, intravenous and general anesthesia, alveoplasty, bone grafts, other oral and maxillofacial procedures. 4) Confidentiality, as well as HIPAA guidelines regarding Personal Health Information (PHI) shall be adhered to and is detailed in the attached Appendix F. 5) The CONTRACTOR will supply CDPC with the information it needs to assure that the services provided by the CONTRACTOR under this contract meet the applicable standards of The Joint Commission, which accredits CDPC. This information includes evidence of primary source verification of required credentials, evidence that the provider is Board certified or Board eligible Oral Maxilla Facial Surgeon (OMFS), and evidence of initial and continuing competence as part of a performance evaluation system. 6) The CONTRACTOR will work under the direction and guidance of the CDPC to coordinate provision of services. Scheduling and supervision of the CONTRACTOR will be provided by the Program Director of the Medical Clinic or representative.
Procedures 1) All patient(s) scheduling shall be coordinated through the CDPC. 2) The CONTRACTOR will track their patients regarding their Oral Maxilla Facial procedures and callbacks and will communicate with CDPC via email or fax. Currently we are using paper consult forms, and OMFS fills out/writes in the report portion of the form that is being placed in section 5 of the paper chart. In addition, the patient’s healthcare provider at CDPC will monitor and follow up with the patient(s). Miscellaneous 1) The privileges of all personnel assigned to provide services to CDPC will be granted by the MSO Credentialing Committee. Upon implementation of the contract agreement and thereafter on an annual basis or upon change in staff assigned, the CONTRACTOR shall provide CDPC with written documentation that will be submitted to the CDPC MSO for credentialing approval. A copy of the CV for all assigned staff shall be sent to the CDPC Clinical Director. According to Section 424-a of the Social Services Law, personnel must be cleared through the State Child Abuse Registry. CDPC will credential the personnel assigned to provide services to the facility and monitor the quality of services based on an annual review process as coordinated by the CDPC Medical Clinic Director. 2) The designated contact persons for CDPC shall be the Medical Director or designee. 3) Halfway through the contract term (initial year and each extended term year) a meeting shall be held between the Clinical Director, Medical, and Dental Director, and the CONTRACTOR to review the contract and determine and/or suggest any need for changes. 4) Individual Malpractice Insurance Coverage shall be provided through the CONTRACTOR. 5) The CONTRACTOR agrees to provide to the facility all information, in a timely manner, needed to assure that the care provided by the contractor at the facility fully complies with applicable standards of the Joint Commission on the Accreditation of Healthcare Organizations, Centers for Medicare and Medicaid Services, and Inspection of Care. This information may pertain to the nature, quality and outcome of services provided; credentials of contracting staff as well as evidence of their initial and continuing competence. 6) The CONTRACTOR will be in compliance with all confidentiality regulation as specified under the federal Health Insurance Portability and Accountability Act (HIPAA) of 1996. 7) Either party may terminate this agreement at any time giving six (6) months notice to the other party. Minimum Qualifications 1) All individuals providing services under this contract must meet the following minimum qualifications:
Site 1) The Contractor will receive direction from the CDPC regarding treatment sites/Contractor’s place of business and as appropriate, to meet facility needs The CDPC reserves the right to stop any work covered by this contract at any time that the CDPC deems the CONTRACTOR to be unwilling, unable or incapable of performing the work to the satisfaction of CDPC. Any work stoppage, either temporary or permanent, would not be ordered by CDPC without prior consultation with the CONTRACTOR, except in the event that the job performance endangers the safety of clients or staff.
